Chronology — dating and quality habits

Each entry should include:
- Date and time (use consistent timezone / “as reported”).
- Source (witness, family, professional, system log).
- Fact recorded — avoid opinion labels (“negligent”) in the chronology body; capture allegations as allegations.
- Action taken and by whom.
- Next review date if applicable.

Avoid:
- Copy-pasting identical text across multiple dates without new information.
- Mixing investigation conclusions into early chronology entries before facts are established.
